Prior to your appointment

Health Intake Forms

Please print and complete the appropriate Intake Forms below.

In-Person Consultations: you may bring your completed and signed form with you to your appointment.

Virtual Consultations (online): please submit your completed form to melliferapnoe@gmail.com at least 3 days in advance of your appointment.

Please note that if you choose to submit the form via email, you are agreeing to sharing your personal and medical information electronically.

Cancellation Policy

Cancellation policy for all holistic and consultation services is 48hours notice. If cancellation notice is received less than 48hours, then a 50% cancellation charge of the service booked will be applied.

*NOTE: Cancellations for a Monday appointment must be made no later than 6:00pm the previous Friday.
